A sad story just gets even more bizarre and sadder. An early scan of Philip Seymour Hoffman’s apartment turned up plenty of evidence that Hoffman had not only relapsed following 23 years of sobriety, he had a wide range of heroin paraphernalia including 50 envelopes, syringes, prescription drugs and empty bags inside according to a source who revealed the information to CNN.

Last year, Hoffman told TMZ that his relapse began with prescription pills before it escalated to snorting heroin. After a short binge, the actor realized his trouble and checked into rehab for 10 days. At the time he thanked, “A great group of friends and family” for helping him find treatment.

Hoffman died Sunday of an apparent drug overdose at age 46.
